Referrals

If you are a GP or psychiatrist wishing to refer a client under a Mental Health Treatment Plan (MHTP), please forward the attached documentation (password protected) directly to enquiry@amybishoppsychology.com:

• Referral Letter, including:

• Client’s name,

• Client’s date of birth,

• Client’s Medicare card number, IRN and expiry date,

• Any diagnoses or symptoms if diagnosis is not yet provided,

• A list of the client’s current medications,

• The number of sessions the client is being referred for

• If the client has previously seen a different psychologist under the same mental health treatment plan and the number of sessions already provided.

• Mental Health Treatment Plan

Any referral letter or MHTP which is incomplete or incorrect will be returned and amendment requested. A client cannot be seen under a MHTP until the amended referral and/or MHTP has been received by our clinic.
